Program uses a clinical team
to provide coordinated treatment services to families identified by DYFS
as having a youth who is at risk of out of home placement. Services
include:
- Intake/evaluation
- Intensive individual, couple and
family counseling in client’s home.
- Support home visits, outreach and
advocacy
- Multi-family support groups
- Information and referral
- Short term treatment planning
- Coordination with other service
providers
